Health & Safety Incident in Ottawa

HIPO Accident: During a scheduled de-energized maintenance operation a worker received an Arc Flash. 

On October 30, 2024, at approximately 11:30 AM, two employees from Plan Group's Technical Service team encountered an arc flash incident during scheduled maintenance operations in an Ottawa building. 

The team was on-site for infrared scanning and de-energized maintenance of electrical equipment. During the removal of a panel cover, an unexpected, sustained arc flash occurred because no breakers or fuses activated. The employee, positioned closest to the panel, suffered burns to his face and forearms. A coworker, who rushed to assist, also sustained burns on his forearms. Both individuals received prompt medical attention at a local hospital and were released later that evening. Fortunately, initial reports indicate they are expected to make a full recovery. 

An investigation is currently underway to determine the contributing factors and root causes. This will allow us to put in place corrective measures to prevent future incidents. 

This event serves as a critical reminder of the importance of adhering to our 12 Golden Rules. Specifically, focus on Golden Rules 1, 3, 5, and 12. While we are relieved there were no fatalities, we must remain vigilant in prioritizing safety in all operations.
